Privacy Policy & Notice of Privacy Practices — Tryozi Group LLC

Who We Are
This notice describes how Tryozi Group LLC ("Tryozi") and the licensed clinicians and medical groups who provide your care handle your information. Clinical services are delivered by licensed clinicians through OpenLoop Health, Inc. and its affiliated medical groups (the OpenLoop Affiliated Covered Entity, including OpenLoop Healthcare Partners, PC and affiliated PCs). Protected Health Information (PHI) created in the course of your care is governed by the providers' HIPAA Notice of Privacy Practices. Where Tryozi handles information on behalf of the providers, we do so under appropriate written agreements and maintain safeguards consistent with HIPAA.
Your Rights
You have the right to: get a copy of your medical record; request corrections; request confidential communications; ask us to limit sharing; get an accounting of disclosures; get a copy of this notice; choose someone to act for you; and file a complaint without retaliation.
You have choices about: sharing with family/friends involved in your care; marketing and sale of information (we obtain your written authorization first); and fundraising (you may opt out).
How We Use and Share Health Information
We may use and share PHI to: treat you; run our organization; and bill for services. We may also share PHI, as permitted or required by law, for public health and safety, research, legal compliance, and similar purposes. We do not sell your information or use it for marketing without your written authorization.
Tracking & Advertising Technologies
We do not send health information — including conditions, medications viewed, quiz answers, or intake/checkout events tied to an individual — to advertising or analytics platforms (e.g., Meta Pixel, Google tags). Advertising and analytics technologies are kept off our intake, quiz, account, and checkout pages.
Your Privacy Choices Under State Law
- Washington "My Health My Data" Act (MHMDA): We collect consumer health data only with your separate, affirmative opt-in consent, and we obtain a distinct opt-in before sharing any such data. We do not bundle this consent into our Terms of Use, we do not sell consumer health data, and we do not use geofences around health-care facilities. You may withdraw your consent at any time by contacting us at the address below.
- California (CCPA/CPRA): California residents have the right to know what personal information we collect and how it is used, to request access and deletion, to correct inaccurate information, and to limit the use of sensitive personal information. We provide notice at the point of collection and do not sell or share your personal information as those terms are defined under the CCPA/CPRA. To exercise these rights, contact us at the address below; we will not discriminate against you for doing so.
- Other states: Residents of other states with comprehensive privacy laws (e.g., Colorado, Connecticut, Virginia) may have similar rights to access, correct, delete, and opt out. Contact us to exercise any rights available to you under applicable law.
Our Responsibilities
We are required by law to maintain the privacy and security of your PHI, to notify you of breaches affecting your information, and to follow the duties described in this notice. We follow state laws where they provide greater protection than HIPAA.

You may file a complaint with us at the contact below, or with the U.S. Department of Health and Human Services, Office for Civil Rights (200 Independence Ave SW, Washington, D.C. 20201; 1-877-696-6775; hhs.gov/ocr/privacy/hipaa/complaints/). We will not retaliate against you.
